TEAM REGISTRATION FORM Team Registration Form Please complete this Registration Form to register your interest in completing in the Mendi 9s 2025 Competition (“the Event”). Completion of this Form does not automatically register your team. Registration of your team will not be confirmed until your registration fees are paid in full and you receive written confirmation from us of registration. Once registration of your team has been confirmed, if your team subsequently withdraws from the Event more than three (3) months prior to the date of the Event, your team may be entitled to a partial refund of their registration fees paid in MD Events (NQ) Pty Ltd’s (“the Organiser”) sole discretion. If your team subsequently withdraws from the Event less than three (3) months prior to the date of the Event, any registration fees paid by your team will be deemed to be forfeited to the Organiser unless otherwise approved by the Organiser in its sole direction. Players and Officials cannot participate until they complete a Deed of Participation, Acknowledgement, Release, Indemnity, Assumption of Risk, Media Consent and Authority (“the Deed”). We recommend that all Players and Officials hold adequate insurance before the Event. A Professional Player is a player who has played five (5) or more games (in total) across any of the following levels: NRL, Super League, NRLW, QCup and/or BMD Cup, UNLESS the player has only played at QCup and/or BMD Cup level (and not NRL and/or Super League level) and has not played at such level for at least two (2) years.
